Aetna Plan Choices in Maine

Individual Standard HMO and
Individual Basic HMO Health Benefits Plans

Aetna Health, Inc. offers two plans in Maine, an Individual Standard HMO health benefits plan and an Individual Basic HMO health benefits plan. You must be a Maine resident to be eligible for these products. If you are eligible for Medicare, you are not eligible for these product.

Northern Maine – Aroostook, Penobscot, Piscataquis, Hancock, Washington, and Somerset

Southern Maine – Cumberland, York, Oxford, Sagadahoc, Franklin, Kennebec, Androscoggin, Lincoln, Waldo and Knox.

To apply for either the Individual Standard HMO health benefits plan or the Individual Basic HMO healthbenefits plan, please mail the completed application along with the first monthly premium payment to:
